How to create forms in digital message webchat bot

Hi team, 

As per our use case, we want to populate form in digital message webchat bot, is there any way to achieve this in pega v8. 8.1

Could you please elaborate a bit more on your use case? What I would need to know is how such a form is being configured, is it part of the case flow or is it prepared by Agent? Where data is expected to be stored?

Such a details will help me to understand the context and if possible, suggest a solution.
