Hi there, the above issue was resolved by just updating the flow action label name from "Transfer" to "Transfer Case".  Also, note can be easily added by using ootb activity -Add history by giving required params as in below snapshot.

Calling this activity in post activity of transfer flow action will add additional note that we can customize apart from the ootb note that is populated as "Performed a Transfer Case". Thanks
