Web Interrogation form issues

When trying to interrogate a web application, the Interrogation Form does not have the select element option in the dropdown which has only three choices and the Create Global Page checkbox is also missing.

using Pega Open Studio 8.0.1016 and Windows 10 Home Version 1803

Any help will be greatly appreciated.
