Can't Display checkbox name in created PDF file

Hi Everyone,

  I try to create PDF file for input screen with OOTB shape. I found it can't display checkbox name in Pdf file. could you give some suggestions for it. Thank you very very much.

Hi Praveen,

Thank you very much. It's same  to my issue!!

Instead of using a check box and a label with inline styling, use the checkbox caption to achieve the same effect.

-> About solution I have used the checkbox caption but not display in pdf.  please check my new attachment. Have Any missing about my checkbox?

Hi Praveen,

 I want ask other about pdf. I want disply the stage in my pdf. 

but in screen it is Arranged horizontally.

in pdf it is Vertical arrangement.

I try for many method but can't display the same as screen.

could you have some suggestion about this?
