This question actually tests your understanding of the Flow rule. Therefore, open the Flow rule and look at the Process tab:
Therefore, to answer your questions:
1) This question is not asking you where the Confirm harness is "used", but rather where is it configured, i.e. referenced. You can see from the screenshot that the Confirm harness is indeed referenced in the Flow rule.
2) The keyword is "always". As you can see from the Process tab of the Flow rule, you can skip the New harness, therefore B is not the correct answer.

First allow me to begin by mentioning that a starter flow is a flow that instantiates a case (work object), like pyStartCase. Starter flows have the "Creates a new work object?" check box selected on the Process tab of the flow rule record.
Response B is false because the New harness is not "always" used. Using the New harness is not required and is an "option." When the "Skip create harness?" checkbox is enabled (see Process tab of a starter flow like pyStartCase), the create harness, typically "New" is skipped and not used. Deselect the checkbox and create a new case. You'll see the first harness presented is now the New harness. It's typically used in scenarios where the user would need to validate the customer before moving ahead and creating the case, such as in a call center situation.

Responce C is a true statement, the Confirm harness is indeed referenced on the Process tab of starter flows (like pyStartCase). It is also true that the Confirm harness is referenced elsewhere as well as in other flow types. The response does not say Confirm harness is "only" referenced in the starter flow, just that it is referenced on the starter flow of a case.
