While trying to upload file in pega pulse, pxRequestor.pyFileUpload is showing attachment name in clipboard, but in UI error comes

While trying to upload file in pega pulse, pxRequestor.pyFileUpload in system pages in clipboard is showing attachment name. But in UI in pega pulse, it is showing error that the file doesn't exist.

Can somebody help to debug the issue. What is the reason for the error.
