Unable to create a case for Onboarding case type

Need a help please, I was preparing for CSA exam and going through the CSA Fundamentals at PDN, while doing exercise on the exercise system provided by Pega, I suddenly got below error which am not able to identify and debug:

This is when I am trying to create a new onboarding case from +Create menu, not able to understand why this problem occured as it was running perfectly 2 days back.

Can someone please help me to fix this problem?
