Sporadic Issue : Child cases getting created without assignments

We are facing a strange issue where child work objects are getting created without assignments.

The logic is we are creating the child WOs by using the pxAddChild activity ...but sporadically we are observing that the child cases are getting created but the corresponding assignments are not.


Any suggestions on this please.
