Pega Platform 8.2 LDAP/AD Integration question

Hi,

We are trying to set up LDAP authentication for our Pega Platform. Found some documentation from other posts on how to go about doing it but am running into some errors. Created an custom LDAP authentication service and configured everything based on previous documentation we found but keep running into authentication issues. Please see screenshot. Thank you.
