Multiple child cases
Is it realistic to create approximately 10k child cases under a parent case?
What locking issues can we get into during child case creation and resolution?

Realistic I don't. Why would you have locking issues on child case creation? I guess it depends on the locking mechanism as well as how you child cases will be created from a design stand point I mean.
