Generally this means that something in your hierarchy is actually unmatched. If you have a dynamic web page where the page doesn't fully reload, you might have match rules that are too specific to each "page". You can use the Refresh Matching button (the orange one) to figure out what control in your hierarchy isn't matched any longer and look for ways to ensure it is matched.
