I got similar issue as well. I resolved by using page unlock method.

Are you calling any activity where it is committing your work object. Generally this happens when you lose the lock on the assignment.

This usually happens in scenario where your case is configured with optimistic locking and someone else has submitted the assignment for the same case elsewhere after you opened the assignment.
Can you tell the exact use case where this is exactly happening?
