Description

Need to be able to send correspondece with the fraud affidavit to the cardholder(CH)/account holder(AH).

Fraud affidavit correspondence: if the user selects the check box for Send Affidavit, an option to send the affidavit file through email or to the mailing address must be available. (E.g. Send via: 1. Email  2. Post/Mail) If the user selects the email option, - Information must be prepopulated into the affidavit PDF that will be attached to the email correspondence (attach email and affidavit PDF to work object) If the user selects the mailing address option, - The PDF must be available with prepopulated data for the user to download (and print). (attach affidavit PDF to work object)  

Note: The fraud affidavit format word document is attached to the story with instructions to pre-populate data. The sample PDF document provided is just a reference to adopt the style
