Assignment mismatch error during pega call transfer

Hi ,

When a call is transferred ,we are seeing Assignment mismatch error in the transferred person Worklist .We are using Pega OOTB call feature .When we  traced we saw the AppTransferWork (ChannelServices-Device-Phone-UI) is getting called and internally it is calling ReassignToWorbasket which moves the assignment to transfers WB and then to person's WorkList .

Can you please suggest what might cause this issue.
