Not confusing from a logic perspective, I think it's wrong though.
The start of the sentence should read "if not selected"
IF
"not selected" (Operator not available to receive work)
AND at runtime a routing activity attempts to send an assignment to this operator
AND CheckAvailability is turned on on the routing activity
THEN
IF
Substitute Operator exists
THEN
Send to the substitute
ELSE
Send to the manager of the operators workgroup
ELSE
Send to the operator

I believe the below statement should be rectified to make it more clear.
"If selected and at runtime a routing activity (with parameter CheckAvailabilityset to true) attempts to send an assignment to the worklist of this Operator ID, the substitute operator if any is used instead"
It simply means that, if "Operator is available to receive work" is selected, then the assignments will be assigned to the Operator's worklist. If its not selected, then it will be assigned to the Substitute Operator's worklist(if any).
