Submit button is not working in DWA

I am implementing DWA.

Facing one issue, when I click on survey link that is send in my gmail id. I am able to fill the details in the survey form. But when I click on submit button. Then I am not able to see Thank message screen. The browser get closed automatically on the click of submit button. 

I follwed the steps mentioned in the below link
