SAE 8 Solution Build, Customer Information fields not shown

Hello,

I am stuck at this exercise, currently at US-107.

When I run it, the Log in step executes fine, but at the Collect Customer information page, it does not show the customer fields. When I started the exercise it used to work, but then I added some functionality, and now it is not showing the fields.

Maybe I added some visibility action sets which causes this, but I can not find it, sorry I am a newbie.

Please help me to find the error.
