Fail Safe / Fail Secure in Access Controls

In a recent check list on vulnerability assessment questions regarding platform security, below question was raised and I was unable to find any PDN article, PDF related to same. Can anyone help on this, where this has been clarified.

"Access controls are implemented in a fail-secure mode, which will not allow access to the system when the authentication is not successfully completed."
