platform changes - tracking

Hello,

Please help in finding details on how to track all the changes that happened on the platform. As developers/testers/admins make changes - somehow the platform can get unsable/instable due to changes, Administrators need to know what changes went into platform to identify and revert the changes.

This will greatly help. Any response is greatly appreciated.
