Case Lifecycle Management Solution Incomplete

The solution for the case lifecycle management lesson appears to be incomplete. The form for the Setup Payroll step includes nothing but a "Submit" button.

Every solution is not necessarily a finished application. A lesson-specific solution only needs to demonstrate the concepts taught within that lesson.
