Icon with label for TextBox, Drowdownlist, Checkbox, Optionlist etcc

We are having a requirement that every field's label in the screen will have unlock icon. The icon will be place at left side of the label. Those fields can be textbox, dropdown list, option list, checkbox, Auto complete control etc.. When user click on unlock icon, the icon will be changed to lock icon and has to run Data Transform/Activity. There are will be over 40 fields and these fields are grouped into three tabs. Ref attachment for a sample field.

Please advise how to implement this user interface requirement with efficient manner.

When click on Lock icon, DT/Activity will insert/update data in PageList. When click on unlock, DT/Activity will update data in PageList. This page list will be applied to another screen to determine whether the field will be read-only or not.
