How to retrieve and show all data from value list or page list in a case

Dear all

I am required to retrieve and show all data from value list and page list in a case.

How do I retrieve and show them? In page list, they have different index, so i wonder if it is possible to get the values..

Please see attachment for the details..

Below link is almost the same that I would like to, but there is no conclusion and I am not sure why they recommend to create declare index.
