@JSUMANTHR16788018 use a link and on click pop up a modal dialog to display terms and conditions (use paragraph rule within the modal dialog flow action section). 

Use a check box to allow user to enable/disable checkbox, I agree all terms and conditions.

Hi @JSUMANTHR16788018: I hope you need a paragraph and checkbox in the create stage. This should be a simple UI. Yu can create it as a common view / can create it specific to your case. To adopt this change, in the create form, follow below.

• Go to case designer and choose configure view for create stage
• Option (whether to reuse a existing view or to select individual fields), add the respective view or fields. Create caption for your checkbox field.
• Navigate to the validation tab of configure view, add validations for the checkbox field
