Reusing data Transform for BackGround Check

When i add "Position" page property, fields are automatically appearing. But for CreditReport,Bankruptcy and Criminal Record the fields are not automatically displaying . can any one help me in this.?

You have to add the objects below to the configure form (make sure they are an object):

 Bankruptcy – Object
 CreditReport – Object
 CriminalRecord – Object
