Where does PRPC get the value of pxIndexCount?

We have an incident where the a duplicate index record was created. All the keys were the same except for the pxIndexCount, and I want to know where this value is coming from. This incident only happened once, but I want to understand how this can be possible and if there's a way to avoid it. Thanks!
