@rnikkoprado I would like to look at the 19.1 version your form. Is it possible to attach that file (the .os file) to the post? I suspect there is something about it that is not upgrading properly. If you cannot attach it to the post, I suggest you open a support request and provide the file to our support team.

One thing you can try on your own though is to copy the upgraded form to a new empty project in 22.1. See if the form loads in a new empty project. If it does, then there is likely something you are doing in your automations that is preventing it from showing properly. I cannot see all of the controls within the Palette on your form, so I cannot see if there are any uncommon types. If it consists of all common types, I would be interested in seeing the file myself.
