How can I edit an attachment of type doc and save it back ?

Please suggest how to achieve this?

Assuming you are talking about work attachments :

There is no straight way to achieve this currently. Max you can use is EditAttachments OOTB flow action but it only works in IE and after saving the word doc adds another attachment as save as copy.

There are some places where we started doing this using Silverlight you might want to expore more.
