Facing issue with Predictive model triggering from flow

We have created a prediction model and trying to trigger it from decision shape in a flow. During execution flow is triggering alert (error attached from tracer) and going to problem flow.

Individual execution of Prediction model is working.

In flow execution output of prediction model is coming in parameters but execution of prediction model is throwing error attached. Please suggest.
