"Couldn't create OMElement from string value" for Connect-SOAP

When we call SOAP connector we're getting this error in Pega log.

What could be the root cause?

& in request header is not getting converted to amp so we are getting connector exception when calling the service.  We imported this service again in Dev and mode is now converted to literal. And if there is any & in the request header it is getting converted to amp and service is running successfully .
