Case action area buttons are disappearing immediately

I am facing a very strange behavior. On the latest case type I have added to my application, case action area buttons are disappearing immediately after I open the flow action. Also before disappearing, buttons are displayed on the case header for a second. You can see the attachments. How can I resolve this ?

Do you have that same issue on any actions or just a specific one? How about with default user on a default case with a default CSS still the same issue?
