Action Insights not getting populated in 8.7.2 version

I am on Pega 8.7.2 version. After deploying code to Test environment with 1 proposition and contact policy. The contact policy is to "Track 1 sent on SMS for the action to block it for 90 days.".

In Test environment, it sometimes inserts records correctly into action insights and sometimes not at all.

But after re-running ProcessInteractionHistory dataflow manually the records get inserted. I have my aggregate dataset ( ActionOutcomesByChannel) materialized as well.

And i dont see this behaviour in DEV. Am i missing something?
