Problem wiith Data table while using decision shape in Pega 8

I have created a decision table and trying to use it in a flow. The decision table has 3 conditions. I am getting the below errors. Please help me out.

Condition Type—

Condition—
Unable to open an instance using the given inputs: Rule-Obj-When SBBk-FW-SBBKInsu-Work.a
Condition—

Condition Type—
