You can easily create this by creating a Report Definition rule and selecting the Patient ID and Total Paid columns.  When you run the report you can right-click on the Patient ID and choose to summarize by that column.  If you save the report and go back to the Report Definition rule you can simply add the Total Paid column and select Sum from the SUMMARIZE column.  Here is a screen shot of the report defintion:
