Clarification related to a question in security module

In the quiz of 'External Authorization' one of the question is as below
As per the lessons I have read, I found out that 'Operator on the fly' means during authentication if the operator does not exist in the system, we can create the operator from a model operator id. But as the answer of the above question is false, can you let me know what else used to create other than operators during 'Operator on the fly'?

We can use it to create an operator on the clipboard for authorization during that session. The authentication activity can also be configured to create an operator ID record in PRPC.

I believe it is not just creation of operator. sometimes if operator already exist then update might also happen on the fly. for example in case a particular operator gets promoted then LDAP might have updated attributes (if we have done the mapping of that attribute) then it might get updated in PRPC operator record also.
