I was looking at the possibility where Pega can store the case attachments (either during case creation if external repository is used instead of database or using attachment migration wizard) in the same hierarchical fashion as for Archived cases in v8.6/v8.7 (where multiple folders are created based on the archival date)

Today attachments are not stored in the hierarchical fashion in the repository and this leads to a performance issue in production where the case/attachment count is very high and all the attachments are stored in the same folder.

For more information, INC-244843 can be referred to where support engineer has clearly mentioned “All the attachment for a particular application gets moved to one folder in the repository by default. There is no way to change it or bifurcate it”.

There could be a custom solution as described here - https://support.pega.com/discussion/how-dynamically-change-attachments-storage-directory-when-using-repository but could this be considered as an enhancement request as many customers might face the same problem if they are storing case attachments to external repository.
